Dr. Coreyann Poly is a Registered Dietitian, Certified Diabetes Educator and Certified Medical Exercise Specialist with over 20 years of experience in medical and community nutrition. She is an expert consultant for culture change, survey preparation and medical chart review in the long-term community.

Dr. Poly is an avid writer and sought after speaker on various topics of adult and older adult nutrition. Her dynamic presentations inspire and educate audiences, speaking at multiple venues including national conferences and local communities.

Dr. Poly maintains an active private practice providing one-on-one individualized nutrition counseling to clients with a wide range of nutrition concerns. Her success lies with her innovative approach to counseling and her passion for nutrition and wellness.

2019 Diet & Dining Manual

3RD EDITION READY THIS FALL 2018 – FOR THE JANUARY 2019 LAUNCH

This is the third edition by Dietitians of New England of the Diet and Dining manual for patients and residents in extended care facilities. Written in collaboration with experts in many areas of nutrition, menu planning, chronic disease management, dysphagia and nutrition support, to create a useful diet manual that can be directly applied to the chronically ill and aging population. The guidelines for diet modifications provided in this manual are supported by the latest in nutrition research and are in line with the culture change initiatives by the Pioneer Network.

Several new features have been added to this edition including: The addition of the International Dysphagia Diet Standardization Initiative (IDDSI), consistency guidelines working to provide a homogenous approach to terminology of diet consistencies for dysphagia therapy. Other additions include The FODMAP diet, Ostomy guidelines, Neutropenic precautions, and diabetes update to pharmacologic management, injectables, and guidelines for severe hypo/hyperglycemia.
